- Change the Nominee of my policy

A nominee is the person who would receive the sum assured or benefits in the eventuality
of the policyholders' death. During the tenure of the policy you could change your
nominee or the percentage he should receive.
- Complete the endorsement request form with the name, date of birth, relation and
percentage of the amount, of the nominee
- If you are nominating a minor, appoint a suitable guardian for the minor, by filling
in his details. The request needs to be signed by the guardian also.
- The duly signed nomination form needs to be signed by a witness.
- Courier the request along with required documents to our Corporate Office for further
processing
- Change my Premium Payment Mode

Premium payment could be done through a cheque or demand draft, direct debit from
your account or from your credit card.
- Fill up the endorsement form.
- If mode of payment has to be changed to direct debit then fill up the direct debit
mandate instruction
- Courier the completed request form with client id and policy number, the debit mandate
and the relevant charge slip to our Corporate Office
- Change my Premium Frequency

You need to:
- Submit a duly signed endorsement form from yourself and a witness.
- Please mention the revised premium frequency - monthly, quarterly, half yearly or
annual mode
- If you chose monthly, the payment is only through direct debit or ECS mode.
- Complete the direct debit mandate instruction form, if you chose monthly payment
of premium.
- Courier the completed request form along with debit mandate (if required) to our
Corporate Office

Sum Assured refers to the amount payable upon death of the policyholder as specified
in the Policy Schedule.
You (policyholder) can accordingly opt for reducing / increasing the sum assured
depending upon the terms and conditions of the policy
Condition:
- We can change the Sum Assured provided the request is received 30 days prior to
policy anniversary
Requirements:

- If the policy holder is different from the life insured then form should also be
signed by life insured.
